Office Hours:

Monday – Friday: 9am – 5pm

Same-day appointments and house calls ($100 travel fee applies) available. 

 

Emergencies:

Call 911 or visit your nearest ER.

 

Communication: 

Phone calls received after 5pm will be returned on the next business day.

If a phone call or email requires more than 5 minutes to reply, we will kindly ask you to schedule an appointment.

Medical issues (refills, or test results) will be addressed during appointments only.

 

Fees: 

All fees are collected before scheduling appointments.

Monthly membership fee is charged automatically after signing up, and may be cancelled with 30 days’ notice.

Late cancellations (less than 24 hours’ notice), missed appointments, or late arrivals will be subject to a $50 fee.
